Effective January 1, 2007, the state of Nevada
began providing assistance with the cost of prescription
medicines to qualified individuals with disabilities.
Eligibility requirements are:

Age 18 through
61 with verifiable disability

Nevada resident continuously for at least
the last 12 months
-
Annual income no more than $26,054 for
singles and $34,731 for couples
Disability Rx Enrollment
for Nevada’s Disability Rx
prescription program is open.
This is an on-line application. You can
fill out the form on-line, print it, and then mail it to the
address provided.
