2018 GMAC Community Needs Assessment

On behalf of the Grants Management Advisory Committee (GMAC), the Nevada Department of Health and Human Services (DHHS) Office of Community Partnerships and Grants (OCPG) is seeking your help to identify future priorities for the Fund for a Healthy Nevada (FHN). To that end, a Community Needs Assessment is being conducted through an online survey, a paper survey, and public forums.


Online Survey

A brief survey has been developed to help identify which services are needed most in communities across the State. The survey can be taken in either English or Spanish; choose the preferred language by toggling the button on the first page, in the upper-right corner of the page.

Paper Surveys

Copies of the paper survey can be accessed by clicking on the links below. Please feel free to print, copy and distribute to your clients, staff, family, and friends.

The last day to complete or submit a survey is Friday, April 27, 2018. If you are returning a paper survey by mail, the address is:

Community Forum Flyers

Results will be tabulated by OCPG staff in early May and brought to the June 14 GMAC meeting, where the committee will adopt recommendations for future funding priorities for submission to the DHHS Director.

This Community Needs Assessment is one of three processes that will be taken into consideration when the State is determining future FHN funding plans. The Commission on Aging (CoA) and the Commission on Services for Persons with Disabilities (CSPD) are also collecting input and will make recommendations to the DHHS Director. This work is being conducted in accordance with Nevada Revised Statute (NRS) 439.630.
